The MDT Meeting is a monthly meeting that brings together staff with the necessary knowledge, skills and experience to ensure a high quality service is delivered to patient’s who clinicians feel would value a MDT discussion. The proposed membership of the Health and Social Care MDT meeting will include staff from the London Borough of Newham, East London Foundation Trust, Newham Health Collaborative, Newham University Hospital (Barts Health Trust), Newham General Practitioners (GP) and Practice Nurses. An MDT makes recommendations and will always take account of the views, preferences and circumstances of patients and their carers when considering its clinical recommendations.


The patients to be discussed at the MDT meeting will include those from the following criteria:

  • 3 or more A&E attendance or hospital admissions in the last year
  • Frequent GP attendances
  • Patients on the palliative care register
  • Patients presented via RAID or Rapid Response teams
  • Residents being supported through Family First services
  • GP and MDT discretion including patients from the table below
