Multi-disciplinary Team Meetings (known as MDTs) are now up and running across Newham, and are coordinated by Newham Health Collaborative.  They take the form of monthly meetings that bring together a broad mix of staff to ensure that a coordinated service is delivered to patients.  The make-up of MDTs can include GPs, social workers, practice nurses, mental health professionals, hospice staff, care navigators and others.  Clinicians can refer patients to an MDT where they feel the patient would benefit from the input of multiple different sort of practitioners.  An MDT makes recommendations for action, and will always take account of the views, preferences and circumstances of patients and their carers.
